Moje Zdrowie: Poland's Free Health Check-Up for Every Adult

Short answer: Moje Zdrowie is Poland's free NFZ-funded health check for everyone over 20: a questionnaire, a six-test basic lab panel plus conditional extras, and a summary visit with an Individual Health Plan. Adults 20-49 can use it every 5 years, over-50s every 3 years.

Poland now runs a free health check-up for every adult covered by the public system - not just children and not just the over-40s. The Moje Zdrowie (My Health) programme, launched on 5 May 2025, gives you a tailored set of lab tests and a sit-down with a medical professional to go through the results. Here is how to use it, based on the Ministry of Health's programme pages.

What the programme is

Moje Zdrowie - formally the "adult health balance" (bilans zdrowia osoby dorosłej) - is a population-wide preventive programme for everyone over the age of 20, funded by the National Health Fund (NFZ). It replaced the Profilaktyka 40 Plus pilot, which was one-off and limited to people over 40; the new programme is periodic, covers younger adults, and adds a full results consultation that the pilot did not include. If you are an expat insured in the Polish public system - through work, study or a family member - you use it on the same terms as everyone else, as described in our NFZ insurance guide.

How often you can take it depends on age:

  • 20-49: once every 5 years
  • 50 and over: once every 3 years

What you get tested

Every participant gets the basic diagnostic panel:

  • blood count (morfologia)
  • glucose
  • creatinine with eGFR
  • a full lipid panel (total cholesterol, LDL, HDL, triglycerides)
  • TSH (thyroid-stimulating hormone)
  • urinalysis

Depending on your age and your questionnaire answers, the extended panel can add liver tests (ALAT, ASPAT, GGTP), PSA for men, an anti-HCV hepatitis C test, and a lipoprotein(a) test - the last one performed only once in a lifetime. Participants over 60 answer three extra screening questions aimed at dementia risk.

How to take part, step by step

1. Fill in the health questionnaire - online through your Internet Patient Account (IKP), in the mojeIKP mobile app, or in person at a primary care (POZ) clinic with staff help. The ministry reports that over 95% of questionnaires arrive digitally. 2. Your questionnaire goes to the POZ practice you choose. The clinic issues a referral for your tests and has 30 days to contact you about an appointment. 3. Do the tests at the clinic or the laboratory it directs you to. 4. Attend the summary visit. A doctor, nurse or midwife goes through your results, your risk factors and next steps: lifestyle, diet, physical activity, vaccinations and further prevention.

The output is your Individual Health Plan (Indywidualny Plan Zdrowotny): a written plan covering recommended oncology screening and vaccinations, plus optional educational advice - including dietetic counselling where your results call for it. Everything lands in your IKP account and, with your consent, is visible to the doctors who treat you later. Participants over 60 can be referred onward for dementia screening.

Why it is worth the hour it takes

The programme is built to catch the big, quiet risks early: cardiovascular disease, diabetes, metabolic disorders, kidney and liver problems, and cancers. It is free at the point of use - the NFZ pays clinics 325 zł per participant aged 20-59 and 349 zł for those 60 and over, so clinics are actively recruiting patients rather than rationing slots. In the first nine days after launch, 80,000 people signed up, with the largest group aged 20-39.

Frequently asked questions

### Can foreigners use the Moje Zdrowie programme?

Yes, if you are covered by Polish public health insurance - employed in Poland, on a voluntary NFZ policy, or insured as a family member. The questionnaire runs through the IKP account that every insured person with a PESEL already has.

Do I need a referral from a doctor first?

No. The questionnaire itself generates the process - your chosen POZ clinic issues the referral based on it and contacts you within 30 days.

I did the Profilaktyka 40 Plus tests. Can I join the new programme?

Yes. The ministry states that participants of the 40 Plus pilot can use Moje Zdrowie once 12 months have passed since their previous tests.

What does it cost?

Nothing for the insured participant. The NFZ funds the programme and pays the clinics per completed check-up, including the summary visit.

What happens if something abnormal shows up?

That is the point of the summary visit: the doctor or nurse explains the result and writes the next steps into your Individual Health Plan - further diagnostics, specialist referrals, or repeat tests on a schedule.
